ITEM SUMMARY:
• This Resolution authorizes the City Manager to execute a professional services contract and any necessary supplemental agreements with Life Extension of Clinics, Inc dba Life Scan wellness Centers to provide Medical Physical Evaluations Services for the McKinney Fire Department.
BACKGROUND INFORMATION:
• The City issued a request for proposal (RFP), 21-69RFP to solicit responses from medical providers.
o Advertised in McKinney Courier Gazette
o Posted on McKinney TV
o 221 matching eBid Supplier Notifications which included seven (7) Historically Underutilized Business (HUB) suppliers.
• Of the 221 supplier notifications, two (2) submittals were received on October 21, 2021.
• A committee comprised of staff from the Fire Department reviewed and ranked the proposals based on the following criteria:
o 40 Points: Program offered including Customer Service

• Based on the results of the evaluated proposals and subsequent interview, the evaluation committee is recommending a contract award to Life Extension of Clinics, Inc. dba Life Scan Wellness Center, the highest ranked respondent to provide these services.
FINANCIAL SUMMARY:
• The Resolution authorizes the City Manager to execute a professional services contract and any necessary supplemental agreements with Life Extension of Clinics, Inc. dba Life Scan Wellness Center to provide medical physical evaluation services in the amount not to exceed $162,703.00
