ITEM SUMMARY:
· One of the goals of the Health Plan for FY 14-15 is to encourage individuals with chronic conditions to stay active in their treatment plan. Starting with Diabetes, the City agreed to pay for testing supplies for those who attended a certain number of Diabetes-related education sessions.
· The overall goal is to successfully manage the disease to avoid progression and other related conditions.
· In setting this plan up with our Prescription Benefit Manager (PBM) MAXOR, Human Resources learned of a company that provides free monitors, monitors the testing of the individuals, and contacts the individual when their results are out of set parameters.
· The cost associated with this contract includes the cost of a replacement monitor if more than one in a year is needed and the cost of test strips which are mailed directly to the employee by MAXOR. All payments by the City will be made as insurance claims through MAXOR.
· This agreement establishes the work relationship between the City's Health Plan, Genesis Health Technologies, L.L.C., and MAXOR.
BACKGROUND INFORMATION:
· The City has an existing contract with MAXOR.
FINANCIAL SUMMARY:
· All expenses will ultimately be paid from the Employee Health Fund and is budgeted.
