· Approve Resolution
ITEM SUMMARY:
· One of the goals of the Health Plan for FY 14-15 was to encourage individuals with chronic conditions to stay active in their treatment plan. Diabetes is one of the first chronic diseases for us to address.
· The overall goal is to successfully manage the disease to avoid progression and other related conditions.
· One of the concerns with Diabetes is multiple morbidities meaning the development of other serious health conditions as a result of Diabetes. Weight is one of those concerns.
· Providing a one-time opportunity to have bariatric surgery and the pre- and post-counseling for greater success is a change we have made to our health plan.
· This agreement establishes the work relationship between the City's Health Plan and Forest Park Medical Center.
BACKGROUND INFORMATION:
· The City's health plan has already been modified to include this benefit.
FINANCIAL SUMMARY:
· All expenses will ultimately be paid from the Employee Health Fund and is budgeted.
