ITEM SUMMARY:
* This Resolution authorizes the City Manager to execute a professional services contract and any necessary supplemental agreements with Life Extension of Clinics, Inc dba Life Scan wellness Centers to provide Medical Physical Evaluations Services for the McKinney Fire Department.

BACKGROUND INFORMATION:
* The City issued a request for proposal (RFP), 21-69RFP to solicit responses from medical providers.
o Advertised in McKinney Courier Gazette
o Posted on McKinney TV
o 221 matching eBid Supplier Notifications which included seven (7) Historically Underutilized Business (HUB) suppliers.

* Of the 221 supplier notifications, two (2) submittals were received on October 21, 2021.

* A committee comprised of staff from the Fire Department reviewed and ranked the proposals based on the following criteria:
